Notification for No-show and Lost to follow-up

CDM Patient definition:
Any person with Form name: CDM Dispensary HTN-DM Encounter Form V0.01(mUzima Form)
Form ID: 514 encounter_type: HTNDMDISPENSARY encounter_type_id: 83 OR

Description;
Notification on muzima for Lost to follow-up and No-show patients.

• No-Show definition: Any CDM patient with none of the above forms filled out for at least 30 days after the booked clinic visit day in the (screening form. formId=509 AND [SYSTOLIC BLOOD PRESSURE (5085) >=140] OR [DIASTOLIC BLOOD PRESSURE (5086) >=90])
• Lost to follow-up definition: Any CDM patient with not seen at clinic 3 months after the booked clinic visit date

User Story:
As a provider, I would like the notifications button to alert me on the number of patients that are No-Shows or Lost to follow-up from my cohort and further allow me to view this list of patients once tapped on. Reminder tag name for each category of the two groups would be good to have example No-show or Lost to follow-up. This will allow me to get notified on who to follow-up from my community unit.

Acceptable criteria.

• On login with internet connectivity, application should run a background check at set intervals on the server to get the no-show or lost to follow-up persons based on definition given above.
• Patients meeting criteria should be added to the device cohort
• Notification button have envelop icon or balloon popup indicating number of new patients meeting criteria for follow-up under the two categorize.
• On selecting the notification button, muzima should open a view of the cohort members requiring follow-up.
• Once CHW form is filled out patient should be removed from notification members.
