Updating Encounter Provider List on mUzima Forms

Currently, encounter providers are hard coded. We must include the pre-defined list of providers on the form in order to be available on the encounter provider field on the form.

Need to update the encounter providers for:

  • include providers from the attachment of "Provider List - Oncology Program 2015-02-18.docx"
