Inaccessible form templates in mUzima

Daniel Malakwen and I have not been able to access the list of mobile form templates available for download in production server. Upon successful download of cohorts the next step on available form templates is blank. Attached is a screen shot of the same. However, we are not able to reproduce the same when we point any of our devices to the cloud test server
