Apply standard form template layout and validation to oncology forms

Currently, only BreastCancerScreeningForm.html and Via.html have met the standard form template layout and validations. These two form templates can be used as the standard ones/guideline as reference when updating other form template (http://173.255.205.23:8082/muzima-form/).

TODO: Apply standard form template layout and validations to:

• Papsmear.html
• Histopathology.html
• Dysplasia.html
