Based on MIH-5, need to create MIH Consent Form Template.

Validations/Rules:

Consent Subject Signature/Name

• It's required.

Consent Contact Purpose:

• It's required.
• If any of the first four is ticked, the last option should be disabled.
• If the last option is ticked, the first four options should be disabled.

Consent Permission:

• It's required.
• If any of the first three option is ticked, the last option should be disabled.
• If the last option is ticked, the first three options should be disabled.
