Create basic registration form with all types of allowed input fields

This ticket is for the creation of a sample registration form that has both the mandatory and optional fields. 

See MFB-2 for a list of the  allowed input fields for a registration form

Update

The form developed should be usable on the OpenMRS instance available at https://demo1.muzima.org. (username admin and password test).
