Create mini clips for mUzima for Health Providers Settings

Here are the list for mini clips under mUzima for Health Providers' Settings:

1. OpenMRS Settings (change of URL, input new username and password) - one mini clip
2. mUzima Settings - nine mini clips
   1. Session timeout (mins) - input values means the app will be logged out within that time value
   2. Automatic form save interval (mins) - input values means the form data will be automatically saved during that time interval
   3. Real time sync - complete form data from device will be sync automatically to server in real time when internet connectivity is available. 
   4. Require medical record number - (check/uncheck) - check means medial record number is required when filling out the registration form template.
   5. Duplicate form data warning - (check/uncheck) - check means when the same form template has been filled with the same encounter date for the same patient, it will prompt a warning message for the user - this is to avoid filling out duplicate form data for the same patient on the same encounter date.
   6. Font size (Small, Medium and Large)
   7. Landing Page (Dashboard and Client List)
   8. Enable SHR - (check/uncheck) - check means it enables to use smart card features.
   9. Enable GPS - (check/uncheck) - check means it enables collecting GPS coordinates when a form template is opened assuming user agrees to turn on device location and internet connectivity is available.
3. Provider/Location Settings - four mini clips
   1. Default encounter provider (check/uncheck)
   2. Manage providers (search server and download additional providers to device)
   3. Default encounter location (select default encounter location which will be auto-injected to mUzima form template during data collection)
   4. Manage locations (search server and download addition locations to device)
4. Cohort Settings - one mini clip
   1. Manage concepts (search server and download additional concepts/observations to device)
