Separate consent question into respective encounter

Whenever the patient gives consent to test contact via mUzima, it needs to have a way to create a new encounter for filling out "CONSENTIMENTO DE TESTE DE CASO INDICE". For the Paciente consente testagem dos contactos observation in EPTS, it needs to include a comment on the obs with "Source=mUzima". This way one can distinguish it's coming from mUzima.
