Include patient address (read only) on Main Tracing Form

This is a suggestion from Damasceno to include the patient address (as read only) on the Main Tracing Form so that Field Workers don't need to open demographic update form in order to know the patient's address info.
Note: I would put this ticket ON HOLD till testing from Tique's team as this suggestion was not part of the scope.
