Add more fields for Inactive Client table

On the CAMPA server side, the Inactive Client table currently has the fields of "N/A", "Study ID", and "Last Post" - see Screen Shot 2017-02-22 at 11.48.00 AM.png. Need to add the following fields:

• Comment - so that Study Staff can add comment on the action actions taken.
• Status - options (New, In Progress, Resolved, Unable to reach client) for Study Staff to choose to indicate the status.
